The claim: Image shows Trump’s leaked hospital form after Butler rally shooting
A Feb. 8 Threads post (direct link, archive link) claims to show President Donald Trump’s private medical records from a July 2024 assassination attempt. It includes an image of an ambulance transport form purportedly from the Pennsylvania Department of Health.
“Turmp’s (sic) medical report from the Butler PA incident leaked,” the post reads.
The form includes alleged details about Trump’s demeanor and medical care.
“Patient is ‘amped up,’ keeps ranting about going on TV again,” it reads in one section. “Treatment Medic administered Ketamine to shut him up.”
It also claims Trump was using cocaine and medications for Alzheimer’s disorder, schizophrenia and erectile dysfunction.

The image is fabricated, the Pennsylvania Department of Health confirmed. The supposed document originated from a satirical social media account.
Pennsylvania Department of Health didn’t treat Trump
The post claims to include details about Trump’s medical treatment after a July 13, 2024, campaign rally in Butler, Pennsylvania. Trump suffered an ear injury at the rally when a gunman opened fire from a nearby rooftop. Two people died, including the gunman, who was shot and killed by a Secret Service sniper.
Trump was taken to Butler Memorial Hospital after the shooting. He was not treated by the state health department, contrary to the post’s claims.
“This form is a fake,” Pennsylvania Department of Health Press Secretary Mark O’Neill told Paste BN. “The department has not been involved with or released any report on the treatment of Mr. Trump.”
Fact check: No, Sen. Warren didn’t say citizens shouldn’t know about government spending
The post originated from the X account GOP Jesus, which shared an image of the document in a July 18, 2024, post and describes itself as “a satirical account for the GOP’s interpretation of Jesus’s will and teachings. Half parody, half commentary, half theories.”
The document in the post is labeled “EMS transfer of care form,” but Trump didn’t arrive by ambulance. Credible news outlets reported Trump arrived at the hospital with a motorcade and walked inside the building.
Other visual clues point to the image’s inauthenticity, including the lowercase p in Pennsylvania on the form’s letterhead, as well as Trump’s dangerously high purported blood pressure of 210/40.
