Whistleblower describes 'perfect storm' at Phoenix VA Medical Center

Dr. Katherine Mitchell talked about a "perfect storm" in the ER and her fears that a patient might die. She later went to the Office of the Inspector General and The Arizona Republic, asserting her status as a government whistle-blower.
